dwc_otg: prevent BUG() in TT allocation if hub address is > 16 #337

A fixed-size array is used to track TT allocation. This was previously set to 16 which caused a crash because dwc_otg_hcd_allocate_port would read past the end of the array. This was hit if a hub was plugged in which enumerated as addr > 16, due to previous device resets or unplugs. Also add #ifdef FIQ_DEBUG around hcd->hub_port_alloc[], which grows to a large size if 128 hub addresses are supported. This field is for debug only for tracking which frame an allocate happened in.

Because keyctl_read_key() looks up the key with no permissions requested, it may find a negatively instantiated key. If the key is also possessed, we went ahead and called ->read() on the key. But the key payload will actually contain the ->reject_error rather than the normal payload. Thus, the kernel oopses trying to read the user_key_payload from memory address (int)-ENOKEY = 0x00000000ffffff82. Fortunately the payload data is stored inline, so it shouldn't be possible to abuse this as an arbitrary memory read primitive... Previously if you did this with fiq_split_enable:
2a) Unplug and replug it lots of times, or
2b) Have a dodgy power supply or cable that caused lots of device resets
then a crash like this would invariably result:
With the patch applied I can now unplug/replug the hub all the way to address 127, whereupon the address assignment wraps back around.
