Heavy updates on the TEP proposal 🛩

- Clean some non-sense and make it as small, simple and clear as
possible
- Make sure the process doesn't block authors to use other means to
write design docs before proposing a TEP
- Add some examples (fictional but based on real enhancements / subjects)
