- librustdoc::clean::clean_lifetime doesn't need a mut doc context
- librustdoc::clean::normalize doesn't need a mut doc context
- move Some() wrapping up into `clean_predicate()`
- simplify nested if in librustdoc::clean::record_extern_fqn()
