Describe the bug
Currently the user is asked to enter the symptom date while creating or updating symptoms but there is no table field for symptoms onset date in the Overview page

To Reproduce
Steps to reproduce the behavior:

  1. Go to Encounters
  2. Click on Overview
  3. Scroll down to Symptoms
  4. See error

Expected behavior
There should be a date field in the symptoms table

@hrit2773 Don't think it's as important as the other columns, might get too cramped. @bodhish Can you confirm ?

@amjithtitus09 then Why are we asking date while adding symptoms. Even diagnoses has onset. Looks incomplete and inconsistent. If its not needed then I guess we should not ask user to enter date.

@hrit2773 On Set date is a critical field for all symptoms, the treatment for cough that is 2 days old and 3 weeks old are different. We should capture it and show it.
