Improved source checking, data tweaks, fixes

• Improved playlist status checking (1)
• Increased media source status checking timeout (from 3s to 10s)
• Added a blacklist for certain broken media sources (hardcoded for now)
• Blacklisted TinyUrl and MP4 source URLs
• Fixed provider playlists not loading correctly from cache
• Fixed channel name matching cache not working correctly after the last update
• Added more channel matching rules
• Tweakes some channels and providers
• Updated the User Agent string
• Write original playlist channel details as tags in the output file

(1): The status of the channels and sub-lists in a playlist will also be checked, as well as their sub-channels and sub-lists, and so on. If no source is found alive, the whole initial playlist will now be considered unplayable. This will avoid many false-positive alive statuses
