Renaming plans to match new pricing page #10862

The new pricing page (.com#3792) is launching soon, and changes the naming of the plans.

Cloud → PostHog Cloud (Self-serve)
Cloud Enterprise → PostHog Cloud (Enterprise)
(Self hosted) Scale → PostHog Self-hosted (Self-serve)
(Self hosted) Enterprise → PostHog Self-hosted (Enterprise)
(Self hosted) Open source → PostHog open source

We should update the plan names where they appear in other places, like:

Anywhere plan names show up in the app
License server/Stripe (@timgl?)
Calendly (@camerondeleone)
??

Anything else on posthog.com can be filed here.
