duplicate provierId + serviceId detected

While testing multiple templates the same providerId and serviceId pair was found from more than one file. This also means one or more templates is not using correct file name, see DCTL1003.

When a file exists that has correct name the invalid named templates need to either change providerId + serviceId content, or be deleted. When none of the templates are correctly named it is up to change requester to decide which one is the correct template to be used with these Ids' and what ought to happen other files.
