With the pathology subtyping analysis, there were decisions made by CHOP pathologists that changed integrated_diagnosis and thus broad_histology and short_histology and these should be updated in some iteration of the histologies file.

Also related are: #607 and #702, in which the reason for changes should be added in the Notes section, for tracking/per request of Adam Resnick.
