How do I debug tool calling issue?

I am trying to figure out why my tools are not being called. I implemented telemetry according to the docs: https://sdk.vercel.ai/docs/ai-sdk-core/telemetry#telemetry

I see Open AI tries to call the function in JAEGER UI

However there is no reason anywhere for why it failed. I have logs in the function that are supposed to be called, but they are never invoked.
Question is, how do I debug the the issue when tool calling is not working?
