Move to jupyter/telemetry

@yuvipanda and @jaipreet-s let's move the design document and initial prototype implementation in #1 to jupyter/telemetry. I think this should be the core telemetry library. We'll revisit the jupyterlab-telemetry repo when we're integrating that library with jupyterlab. How does that sound?

@yuvipanda do you want to port those two PRs, since you're the original author? I don't want to steal credit for them ;)
