Given the markdown:

# Heading

## subheading

```
foo
# bar
baz
```

## another subheading

Calling 'M-x imenu' erroneously shows 'bar' as a heading.

This makes imenu less useful, and is also causing ardumont/markdown-toc#13

This was fixed in #40, closed!
