General instability handling goals in the UI

There seems to be a slew of stability issues handling goals in the web app. These range from cancel buttons in windows not returning to the main form, and it's possible to set goal concern associations based on non-concern assets.

These should be fixed and, rather than switching windows, modal dialogs for dealing with goal refinements and concern associations should be used (to be consistent with obstacle refinement handling).
