notify-sequence-number not included in event notifications

It appears that we are not including the notify-sequence-number attribute in events returned by Get-Notifications or in the message passed to the notifier.

In addition, we should be generating the following attributes at delivery time instead of in the event record:

notify-subscription-id
notify-user-data

Finally, the following attributes MUST be added to the event record:

notify-charset = "utf-8"
notify-natural-language = "en-US"
